Eligibility Condition

Confirm your regular weekly earnings from work have dropped by more than 40% for at least one week for the relevant reason. Confirm a medical doctor or nurse practitioner has certified that the family member requires care or support (Family Caregiver), or is at significant risk of death within 26 weeks (Compassionate Care). Confirm you are providing care or support to the family member.
